Prom Limousine Reservation Contract To follow, you will find our Prom Contract, Customer Liability Agreement, Parental Consent Form. Until you have made reservations through our reservationist, you will not have all of the information required to complete the Prom Contract form. No matter whether you are over the age of 18 or not, but still in high school, we must have a parent’s signature on this form in order to secure this reservation. We must speak to a parent to get the credit card information. We need a copy of the credit card and the cardholder’s drivers license. This allows us to validate the cardholder’s identity. We apologize for any inconvenience that this may cause, but it is for your protection as much as ours. You will also find that there is a liability and parental consent form with this contract. The liability form is a standard form that must be signed prior to transportation of anyone in our limousines. The parental consent form is being required by every person that rents a limousine for usage by minors, who will not be chaperoned by an adult. It was recently brought to our attention that it is a Federal Crime to transport minors across state lines without parental consent. And while we are sure that this will not be a problem, it is better that we get a signed form stating whether this is okay or is not okay with the adult in charge of this reservation. With the crackdown by many of the licensing agencies on companies to ensure their compliance with state and federal licensing, we feel that this is a matter, which must be addressed. Please fax these back to us with the rest of your paperwork. If you feel the need to have each parent sign one or both of these forms, please feel free to copy them and have each parent sign them. However, please know that as far as dealing with Elite, you are the person responsible for this reservation. All other agreements are between you and the other parents. Form #1 Form #2 Form #3 Form #4 All 4 of these forms, a copy of the credit card, and a copy of the credit card holder's driver's license must be faxed or mailed to our office.Once we receive these, we will process the 50% deposit on the credit card, unless there are other arrangements made, in writing between a parent and an authorized representative of Elite Limousines, Inc. If we do not receive these items, you are still responsible for the 50% non-refundable deposit. You have a maximum of 3 days to call and cancel this reservation without penalty! |
